Summary: **Outpatient Chemotherapy and Infusion Center** Oncologist provides outpatient chemotherapy and infusions and is a solo provider. No hospital consults required with low volume remote call. MD can work directly with ER physician or hospitalist for any admissions. No oncology inpatient services at local hospital.

Rate Caps for Submission: Malpractice – Must be included in rates.Orientation – Kicks in after 8 hours. “Per hour of orientation or training beyond 8 hours” Daily – 5000 rate cap – A flat rate for clinic coverage to include Nightly CallNightly Call – INCLUDED IN THE DAILY RATE CAP OF 5000On Call Daily – 24hr remote call by phone – 1500 rate capNO CALL BACK. Provider is not called back in. Remotely called.Holiday – Must be 1.5x the regular rate. “Per holiday hour no OT” Please also include a 24hr call/WEEKEND CALL rate which would be by phone only Fri-Sun can be taken from anywhere

Schedule/Availability Requirements:
Start Date: Looking for someone to start FT next year for 6 months and also looking for specific dates below.December 21 2024 – Jan 5 2025
February 15th – February 23rd 2025 April 13th – April 21st 2025 May 25th -May 31st 2025
Schedule: M-Th 7:00 AM-5:30PM with night call then 24hr remote call coverage Fri-Sun

Facility Location: Red Bluff CA
Setting: Outpatient clinic
FTE: 1.0 FTE
Duties: All aspects of Diagnosis Staging Precision Medicine symptom management and Treatment of Hematology/ Oncology Patients. MD oversight for both the Oncology/Hematology clinic and Infusion Services. Infusion Center requires MD onsite for all treatments. Infusion Center provides Chemotherapy Immunotherapy Blood/Platelet transfusions Hormone therapy Antibiotic Therapy Rheumatoid Arthritis injections and Therapeutic Phlebotomies.
Required Procedures: N/A. Bone Marrow Biopsies are referred to outside provider
EMR: iKnowmed Clinic and Cerner Infusion
Support Staff: 1- clinic RN 2-MAs 2-front office clerks 1-quality analyst/billing and 3 infusions RNs.
Reason for Coverage: Locums one year is up

Travel Preferences see attachment for specific travel guidelines:

· Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market. https://www.gsa.gov/travel/plan-book/per-diem-rates
· Air Travel: 750/round trip cap
· Rental Cars: 65/day all in cap including taxes and fees regardless of car class.
· Mileage: IRS Standard Rate
· Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

Credentialing Information:
Timeframe: 30-60 days
Requirements: **MSO requires the application completed within 3 days of being confirmed despite the start date!!**
