Summary: Holy Cross Davis Hospital HCD is looking for a locum OB/GYN provider to help backfill for all gaps in coverage due to the loss of a few internal providers who have moved. Please make sure your the candidate provides the number of shifts per month the candidate can offer to the team on an ongoing basis. Site is looking for the provider to start as soon as credentialed.

Rate Caps for Submission:
Malpractice
– Must be included in rates.
Orientation
– half the hourly/callback rate kicks in after 8 hours. “Per hour of
orientation or training beyond 8 hours”
Hourly – 400-425 – “Per hour not OT”
Holiday – Must be 1.5x the regular rates “Per holiday hour no OT”
On Call Daily – 3151-3300 – “Per 24 hours to include 4 hours gratis
On Call Holiday Call Rate – 4726.50-4950 – “Per 24 hours to include 4 hours gratis
Call Back
– 400-425 – “Per hour of
patient contact hours after gratis hours are completed gratis prorate to MSA
terms.”
Holiday Call Back – 600-637.50 – “Per holiday hour of patient contact
hours after gratis hours are completed gratis prorate to MSA terms.”
Average worked Callback Hours: Low callback expected
Schedule/Availability Requirements:Clinic hours are Mon-Friday 8a-5p. Clinic coverage needed is Mon Tues amp Thurs – 8a-5p.Wednesdays include 24 hour call surgery assist coverage 7a-7a. 24 hour call includes Mon Wed amp 2 weekends Friday 7a – Mon 7a per month.

Setting: Outpatient inpatient Emergency Room Clinic CallDuties: OBGYNDays per month: 7 shifts per month needed.Required Procedures: Will include Gyn and OB patients with surgeries at HCD as well as assist in surgery. EMR: EPICSupport Staff: MAReason for Coverage: We had 2 OBGYN MD's move out of the state and we are currently looking for replacement. We need help with call until we are able to employ 2 new MD's. Dealbreakers: Must be available for all functions of the job description.
EMR Required Training:Module learn lessons – Must be completed prior to virtual EMR training to attend the training.EMR Virtual training – Only able to attend if Modules are completed before training. Virtual training must be completed before the provider’s first day and is only available Mon-Fri for scheduling.
Travel Preferences see attachment for specific travel guidelines:
· Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market. https://www.gsa.gov/travel/plan-book/per-diem-rates
· Air Travel: 750/round trip cap
· Rental Cars: 65/day all in cap including taxes and fees regardless of car class.
· Mileage: IRS Standard Rate
· Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

Additional Details / Questions Answered by Facility:Admissions per day Patients/day Types of cases Disclosures that may need to be made etc.Total # of beds? 220 total beds at Holy Corss Layton. For call shifts: We have an after hours phone system that will call the provider if they need to speak with a patient. The hospital will also reach out to the provider's cell phone if anything is needed. The provider will need to stay within 20 minutes of the hospital while taking a call shift. How many patients will a provider typically see in a clinic day? Every provider is different however they typically see between 15-35 patients per clinic day. Our locums though normally hae gotten up to 15 patients at the most in a clinic day. Do you have the average call back hours on a call shift? for the call back when on call it varies frequently from 0-2 times.Does this facility have DaVinci robot? Yes they do have the robot.
