Summary: Creighton University Medical Center and University Campus stand alone are in need of locums July – December.
BID DUE DATE: 11/04/24 5pm EST
Rate Caps for Submission:
Malpractice – Must be included in rates.
Orientation – half the hourly/callback rate kicks in after 8 hours. “Per hour of orientation or training beyond 8 hours”
Hourly – 366-410 – “Per hour not OT”
Holiday – Must be 1.5x the regular rates “Per holiday hour no OT”
Schedule/Availability Requirements:
Shifts Available:6:30am-3:30pm11:00am-8:00pm3:30pm-12:30am9:30pm-6:30am

Duties: Routine ER duties codes ER locums don't have a lot of trauma responsibilities

Deal Breakers: Minimum Education: M.D. D.O.Training: Completion of an ACGME/AOA approved Emergency Medicine residency program.Experience: Actively practicing as an Emergency Medicine Physician for the last 12 months or completion of an EmergencyMedicine Residency in the last 6 months
Travel Preferences see attachment for specific travel guidelines:
· Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market. https://www.gsa.gov/travel/plan-book/per-diem-rates
· Air Travel: 750/round trip cap
· Rental Cars: 65/day all in cap including taxes and fees regardless of car class.
· Mileage: IRS Standard Rate
· Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
Credentialing Information:
Timeframe: 60 days completed application needs to be returned to facility 48 hours after receipt
Requirements: Case logs from facilities need to be returned ASAP
Minimum Education: M.D. D.O.

Experience: Actively practicing as an Emergency Medicine Physician for the last 12 months or completion of an Emergency
